Label checklist
Read the label.
Keep your own notes.
Use the same questions for every pack. This checklist organises product information; it does not decide whether a supplement is suitable for you.

Date checked: ______________ Source URL: ______________________________
□ Identify the exact pack
Write down the brand, product, flavour, size and date checked. A different pack can have different ingredients or directions.
Notes: __________________________________________________
□ Read the creatine form
Record the named form. Do not assume a blend or product name identifies it.
Notes: __________________________________________________
□ Separate grams and servings
Record the labelled creatine amount, total serving size, serves per pack and capsules or gummies per serving. Keep unknowns blank.
Notes: __________________________________________________
□ Read all ingredients and warnings
Use the complete pack label for allergens, added ingredients, storage and warnings. An abbreviated comparison is not the full label.
Notes: __________________________________________________
□ Check what testing covers
Does the evidence identify the finished product and lot, laboratory, date and test scope? A supplier claim, ingredient certificate and finished-product report are different.
Notes: __________________________________________________
□ Compare the same price basis
Record the regular pack price and delivery separately. Keep subscription or sale offers separate. A cost comparison is not a dose instruction.

Before a personal decision: speak with your GP if pregnant or breastfeeding, taking medication, under 18 or living with kidney disease.
